Laser Lax for Donor Closure

Abstract Proper assessment of scalp laxity is an important factor in achieving maximum hair volume outcome in hair restoration surgery (Unger et al., editors. Hair transplantation. London: Informa Healthcare; 2011. p. 268–9). To accomplish this goal, surgeons need to evaluate not only donor density but also each individual’s scalp laxity which is dependent upon elasticity and gliding ability of the scalp tissue (Seery, Hair Transpl Forum Int. 11:179–80, 2001). A precise scalp laxity assessment is crucial, especially among patients with extensive hair loss or tight scalp due to the restrictiveness of donor harvesting. Although physicians presently have available several different techniques in assessing scalp laxity, all come with their own specific advantages and disadvantages. Mayer’s elasticity scale (MES) is a popular technique used by hair transplant surgeons (Unger et al., editors. Hair transplantation. London: Informa Healthcare; 2011. p. 268–9). Feldman’s technique requires saline injection into the subcutaneous tissue prior to skin stretch evaluation (Feldman, Hair Transpl Forum Int. 11:119, 2001). The Laxometer, developed by Mohebi and Rassman, relies on two pads with needles to grip the skin for an accurate measurement (Mohebi et al. Hair Transpl Forum Int. 18:161–7, 2008). Wong’s method uses fingers to push the donor scalp skin up and down to estimate the donor strip width (Pathomvanich and Imagawa, editors. Hair restoration surgery in Asians. New York: Springer; 2010. p. 131–2). All methods have found application; however, the MES technique measures scalp elasticity in the horizontal direction, rather than vertically, to the wound closure direction. The Laser Lax is a new light-based instrument that has advantages, in part, due to its practical and reliable ability to provide precise vertical measurement of scalp laxity rapidly and without causing any discomfort to the patient (Tangjaturonrusamee et al., Dermatol Surg. 41(9):1038–42, 2015).
